Legalization of Life Certificates not Drafted at the Consulate
At the Consulate
Complete a legalization application (select the link to get the form) and enclose the following:
- Life certificate form (select the link to get the form) legalized by a notary public, then signed by the county clerk wherever required (New York State), and later authenticated by the Secretary of the State where the affidavit is drafted, and a copy of the said documents.
- Valid passport, Lebanese identification card (I.D.), or family/individual ikhraj kayd and a copy of the said document.
- Fees: $40 (only money orders payable to the Consulate General of Lebanon are accepted).
- Certified self-addressed and stamped envelope for return (no return fees are accepted) if the documents are to be returned by mail.
