Extension of Blue Format Passports for One Year

This service is only applicable for people over 15 years old and under 60 years old, holders of RL Blue Format Passports.

At the Consulate

Complete and sign in person a passport renewal application (select the link to get the form). You should complete the application in full, Arabic and English sections, and provide the following:

• New format Lebanese identification card (I.D. – tazkarat al hawiya) or individual ikhraj kayd, and a copy of the said document.
• Current passport and a copy of its first three pages.
• Three recent passport photos, taken within the past 3 months, with full face and white background.
• Copy of a US driver’s license, utility bill or bank statement for proof of address.
• Parental consent form (select the link to get the form) signed by both parents or legal guardians, either in person at the Consular Section or outside the Consulate and authenticated by a notary public, affirming the parents or legal guardians’ consent to the passport application for minors (under 18 years).
• Fees: $60 for one year (only money orders payable to the Consulate General of Lebanon are accepted).

Kindly note that we only receive mail on working days and during working hours. Also, note that we don’t collect mail from the post office or the offices of any mailing service. Check our holiday and working hours schedule for more details.

Send to the Consulate via certified UPS self-addressed & prepaid envelope for return (no return fees are accepted), a completed passport renewal application (select the link to get the form), along with:

• Copy of your new format Lebanese identification card (I.D. – tazkarat al hawiya) or individual ikhraj kayd, and a copy of the said document.
• Current passport and a copy of its first three pages.
• Three recent passport photos, taken within the past 3 months, with full face and white background.
• Completed finger print form for passport extension (select the link to get the form) authenticated by a notary public.
• Copy of your US driver’s license, utility bill or bank statement for proof of address.
• Parental consent form (select the link to get the form) signed by both parents or legal guardians, and authenticated by a notary public, affirming the parents or legal guardians’ consent to the passport application for minors (under 18 years).
• Signed shipping consent form (select the link to get the form).
• Fees: $60 for one year (only money orders payable to the Consulate General of Lebanon are accepted).
