Apply for a Palestinian travel document

At the Consulate

Complete and sign in person a travel document application. You should complete the application in full, Arabic and English sections, and provide the following:

  • Police report stating the loss/ theft and a copy of the said report.
  • Blue card and two copies of the said document.
  • Where applicable, UNRWA card and two copies of the said document.
  • Where applicable, two copies of the first 6 pages of the lost travel document.
  • Where applicable, foreign passport and two copies of its first six pages.
  • Four recent passport photos, taken within the past 3 months, with full face and white background.
  • Copy of a US driver’s license, utility bill or bank statement for proof of address.
  • Parental consent form (select the link to get the form) signed by both parents or legal guardians, either in person at the Consular Section or outside the Consulate and authenticated by a notary public, affirming the parents or legal guardians’ consent to applications for minors (under 18 years).
  • Fees: $180 for 3 years (only money orders payable to the Consulate General of Lebanon are accepted).

The new travel document should arrive from Lebanon within 120 days from the time we receive your application. When it arrives at the Consulate, the concerned person will be contacted to collect the new document. This should be  either in person at the Consulate or via UPS self-addressed & prepaid envelope, at the applicant’s own risk and after signing and enclosing a shipping consent form (select the link to get the form).

In this regard, kindly note that we only receive mail on working days and during working hours and we don’t collect mail from the post office or the offices of any mailing service. Check our holiday and working hours schedule for details.

Via Certified Mail at Your Own Risk

Send us via email your complete contact information, including full address and phone numbers in the US and in Lebanon, along with scanned copies of the applicant’s:

  • Police report stating the loss/ theft.
  • Blue card.
  • Where applicable UNRWA card.
  • Where applicable, lost travel document.
  • Where applicable, foreign passport.
  • US driver’s license, utility bill or bank statement for proof of address.

Upon receipt of your email, and on your behalf, we will partially complete an application form for travel documents and send it to you via regular mail.

After receiving the said application:

  1. Sign it in front of a notary public.
  2. Sign a shipping consent form (select the link to get the form).
  3. Send to the Consulate the application and the form via UPS self-addressed & prepaid envelope, at your own risk, along with:
  • Copy of the police report stating the loss/ theft.
  • Two copies of the blue card.
  • Where applicable, two copies of the UNRWA card.
  • Where applicable, two copies of the first 6 pages of the lost travel document.
  • Where applicable, two copies of the first 6 pages of your foreign passport.
  • Four recent passport photos, taken within the past 3 months, with full face and white background.
  • Copy of a US driver’s license, utility bill or bank statement for proof of address.

Optional expedited service via DHL Courier express

This service is optional and at the applicant’s own risk, to expedite the issuance and shipping of new passports.

Check the following website Fast Track DHL and take the following steps:

  • Choose the needed service.
  • Pay the shipping fees.
  • Print and provide the Consulate with:
    • Two copies of the shipping DHL service (roundtrip, outbound or return) between the Consulate in New York and the Ministry of Foreign and Emigrants in Beirut.
    • One copy of the applicant’s passport list.
    • One copy of the DHL receipt.
