Shipping of Travel documents will be done ONLY via UPS self-addressed & prepaid envelope at your own Risk.

Apply for a travel document only to travel to Lebanon and only for one trip for a period of 3 months.
At the Consulate, complete a travel document application (select the link to get the form) and enclose the following:
· 4 recent passport photos 1.9×1.4 inch with full face and white background.
· Copy of your Lebanese id or ikhraj kayd issued within the last 10 years.
· Current passport and a copy of its first 6 pages.
· Parental consent form (select the link to get the form) for minors and applicants under 18 signed by both parents or legal guardians, at the Consular Section or authenticated by a notary public, affirming their consent to the application.
· Fingerprint of the right-hand thumb on a specified Consulate form.
· Copy of a US driver’s license, utility bill or bank statement for proof of address.
· Fees: $60.00 (only money orders payable to the Consulate General of Lebanon are accepted).
· If the travel document is to be delivered by mail, itwill be sent ONLY via UPS self-addressed & prepaid envelope, at your own risk (sign this form), while noting that we only receive mail on working days and during working hours. Check our holiday and working hours schedule for more details. Kindly note that we don’t collect mail from the post office or the offices of any mailing service.
If all requirements are met, the travel document will be issued on the same day.
Via certified mail, if you can’t get to the Consulate send us ONLY via UPS self-addressed & prepaid envelope, at your own risk, while noting that we only receive mail on working days and during working hours. Check our holiday and working hours schedule for more details. Kindly note that we don’t collect mail from the post office or the offices of any mailing service:
