At the Consulate, complete a visa application (select the link to get the form)

·         Valid passport for a minimum of 6 months and a copy of its first 3 pages.

·         Copy of Green Card or valid US visa for non US citizens.

·         One recent passport photo 1.9×1.4 inch with full face and white background.

·         Copy of a US driver’s license, utility bill or bank statement for proof of address.

·         Letter of employment stating the purpose of the trip.

·         Letter from the business contact in Lebanon stating the purpose of the trip.

.         Copy of most recent Bank Statement.

·         Fees: $88.00 for single entry visas and $175.00 for multiple entry visas (only money orders payable to the Consulate General of Lebanon are accepted).

·         If the passport is to be returned by mail, ONLY via UPS self-addressed & prepaid envelope, at your own risk (sign this form), while noting that we only receive mail on working days and during working hours. Check our holiday and working hours schedule for more details. Kindly note that we don’t collect mail from the post office or the offices of any mailing service.
